Request for Additional Employment

 

Graduate students with an assistantship are required to submit a signed copy of a request for additional employment. The form must be signed by their respective Graduate Program Director and the Associate Dean of the Graduate School before submission.

International Graduate Student – Request for Additional Employment

Domestic Graduate Student – Request for Additional Employment

NOTE: Graduate Students with an assistantship will NOT be scheduled for work without submitting a Request for Additional Employment form.
